This agreement, between Amanda Cremonini (herein named as THERAPIST) and you (herein named as CLIENT, will begin on the date listed below and will continue on a month-to-month basis at the discretion of both parties, for a minimum duration of 90 days, or 3 monthly billing cycles. PROGRAM DETAILS The THERAPIST agrees to provide the following support to the CLIENT each month for the duration of their work together. One Rapid Transformational Therapy session per month up to 2.5 hours duration in Eatons Hill, or via Zoom. One bespoke hypnotic audio per session for the CLIENT to listen to daily. Recommended daily protocols for anxiety relief for the CLIENT to follow. Self directed weekly email check-ins between sessions. Please note that the THERAPIST may require up to 48 hours in order to respond to emails. The service provided to the CLIENT by the THERAPIST is Rapid Transformational Therapy and Mindset Coaching, as designed jointly with the CLIENT. Therapy and Coaching, which is not directive advice or counselling, may address overall goals or specific conditions in the CLIENT's life. Coaching services may include setting priorities, establishing goals, identifying resources, brainstorming, creating action plans, asking clarifying questions, and providing models, examples, and in-the-moment skills training. TERMS & CONDITIONS OF SERVICE As this is a Case Study, the CLIENT gives permission to the THERAPIST to share outcomes of their Case Study for the purposes of targeted consumer awareness and client attraction activities. A Case Study includes providing summary details of what the CLIENT was experiencing before their treatment, why they came for treatment, how many sessions they have attended with the THERAPIST and what they are experiencing post treatment. Consumer awareness and client activities include: A summary Case Study on the Smiling Hearts & Minds Hypnotherapy website shmhypnotherapy.com.au Snippets shared on social media Snippets shared in mailouts Snippets shared on podcasts Together with any other platforms that may be applicable. The privacy of the CLIENT will be protected to the extent the CLIENT stipulates for the purposes of any and all consumer awareness or client attraction activities. . FEES IN EXCHANGE FOR SERVICES The fee for each month of therapy and coaching is $349, processed automatically every 30 days through THERAPIST’s online payment processor, Strip, for a minimum 90 days, or 3 payment cycles. MONEY BACK GUARANTEE A conditional money back guarantee applies subject to the CLIENT’S full participation in the program including: Attending scheduled monthly Rapid Transformational Therapy and participating to the full extent possible Listening to the hypnotic audio recording daily under guided hypnosis at least once per day Following the recommended therapeutic protocols to the fullest extent possible If for some reason the CLIENT is not satisfied with the services provided by the THERAPIST, the CLIENT may apply for their money back, subject to meeting the conditions outlined above. REFUND POLICY The CLIENT agrees and understands that changing their mind about the Program, failing to follow through or understand the details of the Program, not experiencing the results expected or desired, or experiencing any other similar situations does not entitle a refund. TERMINATION The CLIENT and THERAPIST agree to provide one another with a minimum of 14 days notice if either party desires to cancel further service. Termination notice must be provided in writing via email. CONFIDENTIALITY Subject to the Terms & Conditions Of Service outlined above, the THERAPIST and all representatives of Smiling Hearts & Minds Hypnotherapy affirm that all information provided by the CLIENT will be kept strictly confidential, as permissible by law. PROCESS AND INTENT Throughout our coaching relationship, the THERAPIST will engage in direct and personal conversations with the CLIENT, which will include asking explicit questions and making requests. The purpose of these interactions is to remind the CLIENT of his/her own intentions, and THERAPIST him/her to realise them. In order for our coaching relationship to achieve the maximum result, the THERAPIST asks that the CLIENT agree to the following: 1. Please be on time to all appointments. If you will be late, notify the THERAPIST in advance. If you will miss an appointment, notify the THERAPIST at least 24 hours in advance. Appointments missed without 24 hours notice will only be rescheduled at the THERAPIST’s discretion. 2. Be honest and participate fully. Recognise that our sessions are a safe place to look at what you really want, and what it will take to make it happen. 3. Make a commitment to the action plans you create, and do what you have agreed to do. 4. Understand that the power of the coaching and therapeutic relationship can only be granted by you, and commit to making the relationship powerful. If you see that coaching is not working as you desire, communicate and take action to return the power to the relationship. The CLIENT's signature on this agreement indicates compliance with the above requests, and understanding of the services to be provided. The CLIENT also acknowledges that the THERAPIST cannot and does not offer any guarantees about specific results gained through the coaching process. The CLIENT’S success is strictly dependent on the CLIENT’S adherence to the therapeutic and coaching process, their unique skills, personal circumstances, ongoing motivation, follow through, consistency and perseverance.

Liability I hereby release Amanda Sarah Cremonini, trading as Smiling Hearts And Minds Hypnotherapy ABN 32 420 377 192 , or associated parties, from any liability or claims that could be made against her concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future) by filling out this form. Scope of Practice I understand that Amanda Cremonini is not a licensed physician, psychologist, or medical practitioner of any kind and that hypnotherapy should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, psychotherapist, or doctor. Participation I give Amanda Cremonini, full permission to hypnotise me and to use Rapid Transformational Therapy® knowing that by participating fully in the process and by listening to my personalised recording for 21 days I play an important role in my overall success. Guarantee I understand that although Rapid Transformational Therapy® has an incredibly high success rate, Amanda Cremonini cannot and does not guarantee results since my own personal success depends on many factors that Amanda Cremonini has no control over, including my willingness and desire to affect the changes inside of myself. Audio Recording(s) I give Amanda Cremonini full permission to make audio recordings that may include my voice. I understand that if a recording (or recordings) are made during or after my session(s) Amanda Cremonini retains full copyright over any forms of media that may be produced and distributed to me. Deepening Process I hereby grant permission to Amanda Cremonini to respectfully lift my arm, touch my shoulder, or rock my head during my Rapid Transformational session(s) in order to help facilitate the deepening process. Confidentiality By submitting this form, I consent that Amanda Cremonini may release information to a specific individual or agency if it has been determined that a vulnerable person (child or elder) is at risk; if I, as a client, am in imminent danger to myself or others; or if a subpoena of records has been requested. I also understand that, at any time, Amanda Cremonini may discuss aspects of my case with other colleagues keeping my full name and identity completely confidential always unless I have given permission otherwise.
